Prescription medication errors are more prevalent than most people realize.  Pharmacists and their technicians sometimes fill prescriptions with the wrong medication, or the correct medication but wrong dose which can cause serious injuries. 

Sometimes this happens as the result of a simple mistake by a pharmacist or technician, but often it is a result of understaffed pharmacies with overworked and underpaid employees.  Pharmacies are supposed to have systems of checks and balances in place to prevent errors, but they often fail to follow them or it becomes routine to bypass the safety measures.
